Either. Most clients have me do both, because the handover is where design usually falls apart — spacing drifts, states get skipped, the mobile layout gets improvised. When I build my own designs, what you approved is what ships.
Yes. If you have a logo, palette and type already, I design inside those constraints and extend them where they run out. If the brand itself is the problem, I will tell you that before we start designing pages on top of it.
